We appreciate your interest and enthusiasm in helping Collinsville celebrate our annual Founder's Day activities. By completion of this form, all parties will be aware of what is expected and agreed upon, and you will be added to our vendor contact list.

All concessions, art & crafts booths, games, activities, etc., must be approved by the appropriate chairman, and the Founder's Day committee reserves the right to refuse attendance to any individual or organization deemed not appropriate to the general theme of this special event, or possibly in conflict with similar booths or concessions.

Reservations must be paid in advance, and are due no later than August 11th. If reservation is not received by the deadline, your space, booth, etc., will be made available to another party. You are responsible for any setup you need and for any necessary cleanup from your area.

Please indicate below you choice of booth or concession space, and provide a complete list of what you plan to have for sale, promote, etc., (after unloading, you must remove your vehicles from Main Street to allow those attending to fully participate in the different activities.) If you would like to request the same location you had in previous years, please contact Krista Oslin at 855-4037.
